I, agree to be part of Training and Capacity Building for Power Sector Institutions for Officers of Electricity Regulatory Commissions organised by the Centre for Energy Regulation (CER), IIT Kanpur. I give my consent for video recording of the program and its storage, processing, modification, archival and, hosting at IIT Kanpur and third-party websites/portals. CER in turn offers a commitment to allow the said Recordings to be used appropriately and sensitively.

1. I hereby grant permission to the unrestricted and sub-licensable rights of my name, likeness, voice, image (in video or still), appearance and performance as embodied in the presentation to be used and reproduced in whole or in parts.
2. I give permission to use my name and biographic information to use or promote the Recordings or any derivative work of the Recordings.
3. I understand that this permission signifies that the photographic, audio or video recordings may be distributed in electronic/print editions and in all media of expression, which may include power point presentations, courses and other media.
4. I also give rights to CER at IIT Kanpur to transfer these rights to any other suitable platform in the future.
5. I understand that these Recordings may be used to offer certifications/academic programs.
6. I confirm that I have permission to include the materials which is the intellectual property, including copyright of another person/party, in my session.
7. I understand that the Recordings will be stored, archived, streamlined, uploaded, and made available through a portal/application as per terms and conditions deemed fit by IIT Kanpur.
8. I have no objections in connection with the Recordings which may be copied, used, amplified, published, republished, transmitted, broadcast, displayed, and distributed for any legitimate purpose, including but not limited to distribution by means of streaming or other technologies via the internet, or distribution of audio or video files (e.g., podcasts) for download by the public.
9. The views expressed during the program are the personal views of resource persons/participants. These should not be attributed to organizations/institutions or representations made earlier.
10. I assign all rights related to the Recordings to IIT Kanpur. The rights assigned to IIT Kanpur includes but is not limited to the rights related to the name, likeness, voice, image (in video or still) and appearance of resource persons/participants, and further broadcasting rights.

By signing this release form, I acknowledge that I have read and fully understood this form and give my unconditional consent to have my Recordings uploaded/downloaded.
