I, agree to be part of 16^th Capacity Building Programme for Officers of Electricity Regulatory Commissions, organised by Centre for Energy Regulation (CER), IIT Kanpur. I give my consent for the recording and its storage, processing, modification, archival and, hosting at IITK and third party websites/portals. I further agree and accept the following:

1. I hereby grant permission to the unrestricted and sub-licensable rights of my name, likeness, voice, image, in video or still and appearance to use and reproduce in whole or in parts.
2. I give permission to use my name and biographic information and use or promote the recordings or any derivative work of the recordings.
3. I understand that this permission signifies photographic, audio, or video recordings may be distributed in electronic/ print editions and in all media of expression, which may include PowerPoint presentations, courses and media.
4. I also give rights to IIT Kanpur to transfer these rights to any other suitable platform in the future.
5. I understand that the recordings will be stored, archived, streamlined, uploaded, and made available through a Portal/Application as per the terms and conditions that are deserved to be fit by IIT Kanpur.
6. I have no objections in connection with the recordings to copy, use, amplify, publish, republish, transmit, broadcast, display, and distribute such recordings for any legitimate purpose, including but not limited to distribution by means of streaming or other technologies via the internet, or distribution of audio or video files (e.g., podcasts) for download by the public.
7. I assign all rights related to the recordings to IIT Kanpur. The rights assigned to IIT Kanpur include but not limited to rights related to name, likeness, voice, image, in video or still, appearance, broadcasting rights and performance.
8. By accepting this release form, I acknowledge that I have read and fully understood this form.
